Community Health Assessment

The Healthy Klamath Coalition is pleased to present the finalized 2022 Klamath County Community Health Improvement Plan.

A community health assessment, also known as a community health needs assessment, identifies key health needs and issues through systematic, comprehensive data collection and analysis.

A community health improvement plan is a long-term, systematic effort to address public health problems based on the results of community health assessment and health improvement planning process. Locally, the CHIP is updated every three years following the CHA. Work on Klamath County’s latest CHIP began in October 2022 with the 2022 CHIP published in March 2023.
