Fonemed

After Hours Care

Fonemed is a free telephone nurse service that you can utilize on evenings, weekends, KHP observed holidays and after KHP’s regular operating hours. Knowledgeable nurses can answer medical questions or direct you to other healthcare resources, and our providers and nurses will follow up with you based on Fonemed care notes.



Fonemed nurses cannot perform administrative tasks like making or canceling appointments, reviewing medical records or addressing any non-medical questions regarding KHP’s policies and procedures.

Please call the main line at (541) 851-8110. You will automatically be directed to this service after hours.

Klamath Health Partnership

We are staffed by a comprehensive team of over 50 health professionals consisting of physicians, physician assistants, nurse practitioners, nurses, lab and x-ray technicians, dentists, dental hygienists, and certified ancillary personnel, some of whom are bilingual.

Federal Tort Claims Act (FTCA)

The Federally Supported Health Centers Assistance Act of 1992 and 1995 granted medical malpractice liability protection through the FTCA to HRSA-supported health centers. Under the Act, health centers are considered Federal employees and are immune from lawsuits, with the Federal government acting as their primary insurer. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Tier 4 Patient Centered Primary Care Home (PCPCH)

Klamath Health Partnership has been recognized as a Tier 4 Patient-Centered Primary Care Home.

Patient-Centered Medical Home (PCMH)

Accredited by HRSA, the patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs have been proven to improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. KHP is a 2023 PCMH awardee.

Community Health Quality Recognition

The CHQR recognizes HRSA Health Center Program awardees that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response for the most recent Uniform Data System (UDS) reporting period.
